ASR is the third party administrator (TPA).
Full-time salaried employees may choose between ASR’s PPO plan or High Deductible Plan. The College pays the single subscriber premium for the High Deductible Plan. The College will contribute up to 35% of the dependent premium and you pay the remainder to insure any dependents.
- The PPO1 plan has a $1000/single, $2000/family, annual deductible with a 80%/20% co-insurance split of the approved amount for in-network providers or 60%/40% of the approved amounts for out of network.
- The High Deductible plan (HDHP2) has a $2000/single, $4000/family annual deductible with an 80%/20% co-insurance split of the approved amount for in-network providers or 60%/40% for out of network.
- Beginning 1/1/2025 two additional plans are available

- In Michigan – HAP or Multiplan (typically for Mental Health Providers and Chiropractors)
- Outside Michigan and International – Aetna National PPO Network

Telehealth – ASR/Amwell
Amwell is a safe and secure technological resource that allows you to receive the quality care you need from the comfort of your home or anywhere you are. This benefit is available to health plan participants enrolled on either the PPO or the HDHP and your eligible dependents. The Amwell app offers virtual care for both HDHP and PPO plan participants for $0!Additional information is available – Amwell Flyer
Visit www.amwell.com/cm and click on the Sign Up button in the upper right hand corner and choose ASR Health Benefits under Select Insurance and click continue. Please note that you will need to set up an account with ASR/Amwell and enter a credit card even though there is no co-pay responsibility. When signing up enter “asrcopaywaived” when asked for a Service Key to get a $0.00 co-pay.
